Boarding, Park & Daycare Form

I agree that to my knowledge, my dog is obedient and non-aggressive. I am aware that my dog(s) will be released into the park, off-leash, with other dogs. I understand that my dog must be up to date on all of their vaccines and that my dog must be on flea prevention and parasite-free. If my dog is flea- or parasite-infested, I give permission to treat if necessary. I agree to indemnify and hold harmless Dr. Gary Beall, Dr. Chip Beall, Barking Lot Doggy Daycare, Springboro Veterinary Hospital, and staff from any and all liability of any nature and claims, including any damage my pet may cause to family members or third parties and/or injury, death, sickness, or damage of my pet, family members, or third parties may suffer during or after playgroup participation.
