If you choose an online pharmacy other than our own, please sign the pharmacy release form to receive your refill prescription. Please complete below or click here to download the form and fax it to us, or you may bring them during your visit.

We will gladly sign your online pharmacy prescription request. However, due to FRADULENT ACTIVITY at online sites, we respectfully request that you sign our release form. Our intention is to advocate for you and your pet.*
